Case study: client engagement

Skilled nursing referral admission processing.

Discharge referrals for skilled nursing beds arrive on payer portals, by fax, and by email. The admissions desk had to work across separate logins and spreadsheets. We mapped the workflow, unified intake, and connected documents, summaries, and eligibility with explicit staff review.

What we found

The same referral could exist in three places before anyone answered the hospital.

Portal notifications, fax stacks, and inbox threads each had their own follow-up habit. Spreadsheets duplicated fields the documents already contained. Eligibility checks happened late or in a separate tab. Nothing was wrong with the people on the desk; the process had grown one channel at a time.

Pare

If the referral is one patient, why is intake three queues?

Steps we removed or consolidated

  • Parallel spreadsheets. One row per channel meant status drift between portal, fax, and email.
  • Re-keying from faxes. Demographics and payer data were typed again instead of flowing from intake.
  • Orphan document folders. Clinical packets lived outside the queue staff actually worked from.
  • Late eligibility. Coverage checks ran after the desk had already invested time in the wrong thread.

Steps we kept with a person

  • Clinical judgement. Accept or decline stays with licensed staff, not automation.
  • Packet review. Summaries assist reading; staff sign off on what goes to the record.
  • Eligibility exceptions. Payer responses are reviewed before they change bed decisions.
  • Outbound response. Hospital and family communication stays human-directed.

One queue for intake. Humans stay on decisions that carry clinical and financial risk.

Sync

What survived, we wired in order: capture, queue, documents, eligibility, then response.

Channel intake

Portal events, fax, and email land in one ingestion path. Each referral gets a single record instead of three parallel trackers.

Desk queue

Admissions staff work one prioritized list with status, owner, and SLA-visible aging. Lanes are visible; the desk is not split by login.

Documents and summary

Clinical packets attach to the referral. Structured summaries highlight what staff need for a first read without replacing the source chart.

Eligibility

Coverage checks run against the referral record early enough to matter. Results stay on the chart for review before bed decisions.

Review gates

Major state changes require an explicit staff action. Automation prepares; people confirm.
